RN, Cancer Survivorship Nurse Navigator, Cancer Programs, Full time, Days

Provides leadership in the survivorship program by coordinating at-risk cancer patients and coordinating oncology services to address the physical, spiritual and psycho-social needs of the patient as indicated. Provides patient education, follow-up and referral to appropriate resources. Serves as a resource to patients, their families and staff regarding cancer disease related issues.  Collaborates with other members of the oncology team to assess and address barriers to care and leads in community related screening and prevention events. Assists with data collection, tracking and reporting of clinical outcomes.

Required Experience

• Current California RN license
• OCN required or needs to be obtained within one year
• Member of professional organizations
• Minimum two (2) years of working with oncology patients.
• Experience working with electronica medical records

Preferred:

• BSN
• MSN
• Chemo-certification
• Current BCLS certification
