Responsible for ensuring quality and integrated clinical patient care and physician/nurse relationships, support organizational financial viability, operational effectiveness, patient satisfaction and strategic initiatives. Ensures safe transitions in care through the development and enhancement of strong partnerships with the post-acute care network. Oversees the metrics reflective of effective care management functions and processes.

- Experience or knowledge of Health Plan requirements regarding Utilization Management, Quality Management and processing of Claims, Grievances and Appeals
- Knowledge of regulatory requirements (CMS Conditions of Participation, The Joint Commission, Title XXII, OSHA, etc.) on a conversant level.
Required Experience
Required:
- Active California RN license or Social Work LCSW license
- Minimum of five (5) years’ experience in acute care hospital, case management, social work, utilization review, or discharge planning in an acute care hospital
- Minimum of two (2) years leadership and department management experience.
Preferred:
- Master's degree in nursing, health care administration, business, or related field is also required
- Case Management Certification
